You must provide information for at least one person who owns 10% or more of the business OR has significant control over it. For each person with ownership/control, collect:

  • Country of residence
  • Nationality
  • Tax Identification Number (TIN)
  • Address
  • Supporting documents
    • Proof of identity
    • Proof of address

Ownership Details

  • Designation
  • Percentage of ownership
  • Relationship establishment date
Important: The total ownership percentages across all associated parties must add up to more than 0.01% but less than 100%.
